Please note that the Special Consideration Form has been updated since this semester (Semester 2 2017).
If you would like to apply for any special consideration for your assessments including supplementary exam based on medical reasons, you will be required to provide TOP’s Student Medical Certificate on page 3 signed both by you and a registered medical practitioner.
The medical certificate issued by medical centres won't be accepted and assessed any more.
